Field Trip and Activity Forms and Procedures
Please the attached forms for scheduling an event at LA High or a field trip. For any questions, please see your department or administrator. Below is a link for the LAUSD field trip handbook. The handbook lists all the approved sites for field trips. The handbook is also attached to this page. The link of "REQUEST FOR APPROVAL OF SCHOOL ORGANIZED TRIP" is the field tirp form you need to complete and have signed for a regular field trip.
