ALL STUDENTS REQUESTING A WORK PERMIT

Follow these simple steps:

• GET A JOB OFFER FIRST
• Obtain a B1-1 Form, Statement of Intent to Employ Minor and Request for Work Permit at the Counseling Office or download and print from Forms page)
• Fill out the form completely and properly (PRINT LEGIBLY); obtain signatures from your Employer and Parents.
• Return the application to Counseling Office.
